Affinity Care of Virginia offers palliative and hospice care services across the state of Virginia. Our staff is dedicated to providing compassionate care that treats your loved one as a unique individual and meets their changing needs.

Whether your loved one needs hospice or palliative care, Affinity uses a collaborative approach that considers your family’s wishes. Our goal is to create a comfortable environment for your loved ones, ease their physical symptoms, and attend to their spiritual and emotional needs.

Palliative Care

Our palliative care (CPR) services support your loved ones as they undergo aggressive treatment for their medical condition. Affinity’s CPR team includes a social worker, nurse practitioner, physician, and support staff who develop a comprehensive plan that connects your loved one to resources, helps ease their adverse symptoms, and helps them to live as comfortably as possible.

With Affinity’s CPR services, your loved one receives:

  • Visits and phone consultations by a palliative care nurse practitioner
  • Social worker home visits
  • Around the clock access to triage
  • Care coordination with PCP and Specialists
  • Spiritual and emotional support
  • Connection to community resources

Palliative care is appropriate for persons suffering from life-threatening or chronic illnesses. It allows you to seek the intensive care you need while receiving support for yourself and your family at home. Most private insurance policies and Medicare cover our palliative care services.

Hospice Care

Affinity offers hospice care to individuals with terminal illnesses in Virginia. Family members, who have not faced this situation may ask, “what is hospice care?” Hospice care focuses on providing terminally ill patients with end-of-life support services that improve their quality of life and allow them to pass with dignity and respect.

Our hospice care provides trained staff members who are on call 24/7 and work with the patient and their family to develop a care plan that supports their physical, emotional, and spiritual wellbeing. As part of Affinity’s hospice care, your loved one receives:

  • Pain and symptom management
  • Home health care services
  • Training on how to care for your loved one at home
  • Access to social services
  • Alternative therapies such as reiki, music, aroma, massage, equine, and virtual reality
  • Short-term inpatient care

In addition to these services, our team also provides bereavement counseling to friends and family of terminally ill patients to help them cope with the situation.

NURSING CARE

Regular visits by registered hospice nurses who have specialized training in pain and symptom management.

HOME HEALTH AIDeS

Visits by hospice home health aides provide additional personal care, time, and attention.

Emergency Care

Consultation and emergency care is available twenty-four hours a day, every day of the year.

Psychosocial Support

We provide psychosocial support for our patients, their families, and their long-term staff and caregivers.

Spiritual Support

Constant spiritual support is provided to both patients and their family members if requested

Special Therapies

Physical, occupational, speech, music, and other therapies indicated in the plan of care are all provided by our expert staff.

Volunteers

Trained volunteers who provide companionship and support for our patients and their families.

Bereavement Counseling

Bereavement counseling for family members is provided throughout the patient's illness and for a minimum of thirteen months following death.
