Facing a life-threatening, chronic, or terminal illness is an overwhelming, emotional situation for the patient and their family. Receiving compassionate, professional care during this time is essential to the well-being of everyone involved. The right palliative or hospice care provider can make a difference in the quality of life for the patient as well as provide support for the patient’s loved ones.
One Community Hospice and Palliative Care offers compassionate palliative care and hospice care services for families in the Kansas City area. Our staff is trained in palliative and end-of-life care and works to ensure your loved one is supported emotionally, spiritually, and physically.
What is Hospice Care?
Hospice care focuses on providing support to patients who are suffering from a terminal illness and would benefit from professional care in addition to what their families can provide. Hospice care can be provided at home or in a hospice care facility, depending on the patient’s needs.
Hospice care is frequently used for patients with cancer, Alzheimer’s disease, dementia, congestive heart failure, chronic lung disease, and other conditions which make it difficult to care for themselves.
Although hospice care sometimes indicates that a patient’s end-of-life is imminent, many patients receive hospice care well in advance of this state. Instead, they obtain hospice early on in their terminal diagnoses, allowing them to enjoy an improved quality of life for a longer time.
Hospice care services with One Community include:
- CNA and physician home visits

What is Palliative Care?
Palliative care is specialized care that helps people with serious or chronic illnesses maintain their quality of life while they pursue medical treatment. In palliative care, a team of practitioners, including a physician, nurse practitioner, social worker, and sometimes a chaplain, provide extra support for the patient and their family.
Palliative care is appropriate for any stage of a person’s serious illness. A palliative care plan seeks to provide relief from adverse symptoms caused by your condition, coordinate care between all health providers, aid in emotional and spiritual support, and encourage communication with family members. It also provides access to 24-hour support and home care services and improves your quality of life.
Most insurance policies, including private and public plans, cover some form of palliative care.
One Community Hospice and Palliative Care accept most private insurances to make compassionate care services accessible. We accept Medicare and the following private insurances:
- Aetna CVS Health
- Ambetter from Home State Health
- Anthem Blue Cross and Blue Shield
